Abstract
Uterus didelphys with obstructed hemivagina and ipsilateral renal agenesis syndrome is a rare congenital anomaly of the female genital tract. Most of these patients present after the onset of menstruation with complaints of dysmenorrhoea, lower abdominal pain, pelvic mass and vaginal discharge. A 45 year old woman presented with lower abdominal pain, febrile senses, foul odor vaginal discharge with left cystic pelvic mass. She was diagnosed as a case of uterus didelphys
with communicant obstructed right hemivagina through small fistula and ipsilateral renal agenesis associated with left tubo-ovarain abscess after routine ultrasonography and following abdomino-pelvic CT and MRI. Excision of the right hemivaginal septum was performed after antibiotics treatment of infection. This condition of communicant hemivaginal obstruction should be considered to prevent misdiagnosis or suboptimal treatment and decrease morbidity and unnecessary surgical procedures in these patients.
